Prior to enrolling in a Field Placement, Internship, or Internship in Diagnostic Assessment course, the appropriate application must be completed and submitted to the student's advisor for approval. Complete application packets can be found in the Program Handbooks.

Field Placement Applications Must Include:

1. Field Placement Approval Form
2. Field Placement Preparation Form
3. Philosophy of School Counseling
4. A current resume
5. Proof of Student Liability Insurance
   • Healthcare Providers Service Organization
6. Copy of TB Test Results
   • St. Charles County Health Department
7. Copy of background check clearance. Student should complete a Family Care Safety Registry (FCSR) Background Screening. The clearance letter must be dated no earlier than one year prior to the start of Field Placement.

The following items may be included in the Field Placement Application, but are only required if the school district requires them:

1. Submission of a Request for Criminal Record Check
(within the last six months)
2. FBI Fingerprint Report
   • L-1 Identity Solutions

Internship Application Includes:

1. Clinical Internship Proposal
2. Completed and Signed Clinical Internship Proposal Contract
3. A current resume
4. Site Supervisor's Resume or a copy of Site Supervisor's Credentials as a L.P.C.
5. Proof of Liability Insurance
   • Healthcare Providers Service Organization

Internship in Diagnostic Assessment Application Includes:

1. Internship in Diagnostic Assessment Approval Form
2. Internship in Diagnostic Assessment Preparation Form
3. A current resume

The following items may be included in the Internship in Diagnostic Assessment Application, but are only required if the school district requires them:

1. Proof of Student Liability Insurance

2. Copy of TB Test Results (within the last six months)
