The Student Teaching Application Process will now be submitted through your Foliotek account
in addition to submitting a hard copy of each component.
After uploading all materials of your application, you must present a hard copy of each application component to Ms. Katie Richter, by the stated deadline (September 1 for Spring semester; February 1 for Fall semester)

4. Advising Form – Advisor’s Signature Stating Eligibility – Schedule an appointment with your Advisor for verification of eligibility to student teach - scanned and uploaded.
5. Application for Degree Signed by Advisor– Xeroxed copy - Scanned and Uploaded
6. Passing C-Base Test Results – Undergraduates Only – scanned and uploaded.
7. Passing Praxis Test Results – Scanned and Uploaded.
8. TB Test Results – (1) Copy and dated no earlier than 6 months prior to student teaching – Scanned and Uploaded.
*Undergraduate students may be eligible to receive free TB testing at Lindenwood’s Health Center.
*Fall Student Teachers may upload a letter verifying plans to get the TB test toward the end of February. Requests will not be presented to school districts until those results are submitted.
*TB tests can be distributed through your doctor’s office or through any urgent care, walk-in clinic, or the St. Charles County Division of Public Health.
TB tests through the St. Charles County Division of Public Health currently cost $15 to administer. Visit their
website to schedule your appointment.
9. Signed Letters of Recommendation – (2) - May range from Academic, Employment, and/or Character References – Scanned and Uploaded.
10. FBI/Highway Patrol Background Clearance – dated no earlier than 1 Year prior to student teaching. – Lindenwood receives results.
11. Request for Family Care Safety Registration (FCSR) Background Screenings – Dated no earlier than 1 year prior to the start of student teaching - Results Sent to Student. – Scanned and Uploaded

*If you are already on file with FCSR, follow the instructions to request a free updated copy of your clearances be sent to your current address.
To Submit Your Completed Student Teaching Application, you must present a hard copy of each component to the Student Teaching Placement Coordinator by the indicated deadline.
There will be a Mandatory Student Teaching Informational Meeting the semester prior to student teaching.
Students will be enrolling in the course at this meeting and will be given urgent information regarding the weekly Student Teaching Seminar class.
The Mandatory Informational Meeting is typically scheduled in April for Fall Student Teaching and November for Spring Student Teaching.
If all pending application results and components have not been submitted by the date of the Mandatory Informational Meeting, students will need to re-apply for the next student teaching term.
Foliotek Registration information can be found in the Education Office, Roemer 106.
Access to a free scanner can be found in Butler Library and the computer labs in the Spellman Center.
